The city of your first US hire shapes three things: who you can recruit, which hospitals you can reach and which employment rules apply. For UK HealthTech companies, three cities come up more than any others. They are Nashville, Houston and Boston.

Each one suits a different kind of company. Nashville works best if you sell to hospital operators. Houston is strong when you need clinical partners at scale. Boston makes sense for research-led companies hiring scientific talent. The state rules behind each city also differ more than most founders expect.

 

Four questions to answer before you pick a city

Most UK founders start with the map. It works better to start with your commercial plan. Before you compare cities, be clear on these four points:

  1. Who signs the contract? A hospital operator, an academic medical centre and a research partner often sit in different places.
  2. Where will you prove the product works? Pilots and clinical studies need partners who are close enough to visit often.
  3. Who is your first hire? A commercial lead, a clinical specialist and a scientist are found in different talent markets.
  4. Which rules will you live with? Tax, leave, pay transparency and non-compete rules are set state by state.

Your answers usually point to one city quickly. The sections below show how each one stacks up.

 

Nashville: built for selling to health systems

Nashville is one of the main centres of the US healthcare business. Its health care industry has an economic impact of $72.1 billion, according to the Nashville Health Care Council. The region is home to more than 900 health care service providers, and HCA Healthcare has its head office there.

That makes it a strong base for companies selling workflow, revenue cycle or patient access tools. Buyers are concentrated, and executives are used to meeting vendors. Vanderbilt University Medical Center adds an academic partner for companies that need clinical input as well.

For a UK company, the attraction is proximity to decision-makers. A commercial lead based in Nashville can build relationships with buyers face to face, rather than flying in for every meeting.

Employment rules

Tennessee has no state income tax on wages, so there is no state withholding to run. There is also no statewide paid sick leave law. You will still need to register for state unemployment insurance before your first payroll.

 

Houston: clinical scale and the Texas Medical Center

The Texas Medical Center describes itself as the largest medical complex in the world. It records 10 million patient encounters a year and employs more than 120,000 people. Houston also jumped three places into the top 10 US markets for life sciences R&D talent in CBRE’s 2025 ranking.

Houston suits companies that need clinical partners, trial sites or large patient populations for validation. There is also a direct UK link. The Texas Medical Center and the UK government signed a BioBridge partnership in 2018 to help British life sciences companies enter the US market.

Dell Medical School in Austin is also an ABHI programme partner, so the Texas network reaches well beyond one city.

Employment rules

Texas has no state income tax on wages either. Two rules catch UK companies out:

  • Workers’ compensation is optional for most private employers. Employers without it must tell the state and report certain injuries.
  • Texas SB 1318 limits non-competes for physicians, nurses, dentists and physician assistants. For agreements signed or renewed since September 2025, restrictions are capped at one year and five miles. They must also include a buyout capped at one year’s salary.

If you plan to hire clinicians in Texas, check your contract template before you send an offer. A UK-style restrictive covenant is unlikely to meet these rules.

 

Boston: research depth, at a price

Boston-Cambridge widened its lead as the top US market for life sciences R&D talent in the same CBRE report. It holds nearly 13% of core R&D roles nationwide and ranks third for medtech talent. For research-led companies, that depth is hard to match.

The trade-off is competition. Your first scientific or engineering hire will be weighing your offer against well-funded local employers. Budget for that before you advertise the role.

Employment rules

Massachusetts is the most regulated of the three states. Employers with 25 or more employees in Massachusetts must include pay ranges in job postings. This has applied since October 2025, including to remote roles based in the state.

The state also taxes wages and runs a paid family and medical leave programme. Employers must offer up to 40 hours of earned sick time a year, paid if they have 11 or more employees. Once you have six or more employees, you also pay the state’s Employer Medical Assistance Contribution.

Non-competes in Massachusetts are limited to 12 months in most cases. They need garden leave pay or other agreed consideration, and cannot bind non-exempt employees.

 

Hiring nurses and clinical staff across state lines

Many HealthTech companies need a nurse or clinical specialist to support pilots in several states. Licensing decides how easy that is.

Tennessee and Texas have both been part of the Nurse Licensure Compact since 2018. A nurse with a multistate licence from either state can practise in other compact states without a new licence. The compact now covers 43 jurisdictions.

Massachusetts passed the compact in 2024 but has not implemented it yet. The state is targeting May 2027, so Massachusetts nurses cannot get a multistate licence for now. If your clinical team will travel, this can favour a Nashville or Houston hire.

Your team probably will not sit in one city

Many UK HealthTech companies hire a clinical lead in one state and a commercial lead in another. Each state where someone works needs its own payroll and tax registration. A Delaware entity does not cover this, as we explain in Your Delaware Entity Isn’t Enough.
